NHS North Lancashire was established in October 2006 following the reorganisation of PCTs in England. A coming together of the former Fylde, Wyre and Morecambe Bay PCTs, NHS North Lancashire commissions healthcare for its population of 340,000 residents from a budget of around £500 million per year. As a commissioner of health care and a provider of community services our approach to VTE is multi-dimensional: - To raise awareness of VTE across our health community and population through clinical leadership and social marketing.
- To commission VTE risk assessments for all of our patients entering hospital and ensure proper audit and outcome measurement. We will use the Map of Medicine VTE Risk Assessment as our reference.
- To improve discharge information and follow up.
- To improve medical management of VTE across our PCT area. For example, we have two major acute providers using different fractionated heparin regimens.
- To promote better primary care/community management of VTE. This will begin with a PRIMIS search of all of our 39 GP practice systems.
- To improve information systems at provider and commissioner levels to enhance surveillance of VTE.
- To explore innovations in the assessment and management of VTE. For example, the role of new medications and novel assessment instruments.
